Our Outreach Service provides flexible, short‑term or ongoing support for individuals who live independently but require additional guidance to maintain stability and wellbeing. This includes practical assistance, wellbeing checks, guidance, and encouragement, helping service users build confidence and life skills within their community.

Outreach support can reduce isolation, prevent crises, and encourage positive lifestyle changes while promoting independence.
